Please note: although we do have a full time office manager, we are not a crisis help line and our phone line is not monitored 24-7. The best way to cancel or reschedule, or resolve technology issues for online appointments is to contact your provider directly. Our office manager is often on hold with or speaking with insurance so response times will vary.

Client Portal
  • Your therapist’s email address is available on their biography page under “Our Team”. You may also contact your therapist via the Therapy Notes portal. Here is the link.

  • If you want to cancel or reschedule your therapy appointment, please contact your provider directly via Therapy Notes messenger or cancel directly on the portal. Here is the link, or send your therapist an email.

    Reminder if you cancel within 24 hours of your appointment you will be charged our cancelation fee.

  • If you wish to contact your medication provider, please do so through the Therapy Notes portal. Here is the link.

  • If you want to cancel or reschedule your medication appointment, please do so directly via the Therapy Notes portal. Both Tessa and Stacy’s schedules are up to date and this is the fastest way to reschedule or schedule an appointment. Here is the link.

  • If you have prescription refill requests or questions regarding medications, please contact your medication provider through the Therapy Notes portal here.

  • Your dietitians’s email address is lauren@rootedandrestored.org or you may message Lauren directly via the Therapy Notes portal. Here is the link.

  • If you have concerns regarding your copay, a change in insurance, coordination of benefits issue, etc: please contact our billing department at billing@rootedandrestored.org. The best way to reach us is via email and we can schedule a time for a phone call if you prefer.
